Timetable for the North Wales Integrated Autism Service Drop-In (Social Hubs)

The family transition service runs across North Wales to support young people with learning disabilities 14-25. We offer youth clubs, 1 to 1 person-centred planning sessions and information access related to transition. We work to support families through a variety of transition stages. We have had success in hosting informal chat to support parent to meet, discuss their concerns about transition to allow them to build a community network. This has proved successful, increasing member resilience and helps us as a team to direct our support.

We are hosting transition talks for parents to discuss their concerns or tips around transition in Flintshire. It is free for parents to join.
